DO I NEED TO SUBMIT HEALTH CLEARANCES?

If you will be taking only the online HAW 101 class, you do NOT need to submit health clearances.

​

You do not need to submit health clearances if you will be taking only online courses (meaning there is no in-person component to the course).  However, if you will be taking any courses with an in-person component, you will need to submit your health clearances in order to be able to register for those in-person (or hybrid) courses.

WHAT RECORDS DO I NEED?

You must submit records of the following:

​

• TB Test at age 16 or older.

• MMR Immunization, minimum of two (2) shots.

• Varicella (Chickenpox) Vaccincation

• Tdap Vaccination

HOW DO I SUBMIT MY HEALTH CLEARANCES?

 

We strongly recommend using File Drop to submit your documents, as this is both secure and a fast way to send to the Health Center.  See below for instructions.

File Drop (Secure email)

You will need web access and have your health information stored on the device that you're using (computer, phone, tablet, etc).

1. Go to https://www.hawaii.edu/filedrop

2. Login with your UH username and password

3. In the recipient field, type: leeshc@hawaii.edu

4. Check the box for Require Authentication

5. Please include your UH ID# in the Comment field

6. Click the Choose File button to browse for your medical records

7. Click the Start Upload button

8. You will be redirected to a confirmation page with the link to your uploaded file

9. You should receive an automated reply confirming the upload went through
